Samet asks that all subcontractors complete a pre-qualification application via TradeTapp by Building Connected once a year. This system mainstreams questions, utilizes multi-users to complete the application, and saves all documents on a secure server by Autodesk.

  • Enterprise business certification proof (if applicable)
  • Professional licenses proof (if applicable)
  • Minimum of 3 project references
  • Current most fiscal year-end financial statement
  • Audited or CPA reviewed statements preferred (consists of a balance sheet + income statement) *
  • Current year W9
  • Letter from your bank stating your relationship
  • EMR Verification
  • Letter from vendors insurance agency stating their EMR score for the last 3 years *
  • 3-year loss run report from vendors insurance agency if EMR is over 1.25 * , if you don’t qualify for EMR, or if business is less than 10 employees
  • OSHA 300 & OSHA 300A logs from the last 3 years
  • Current safety manual
  • Current substance abuse policy
  • Job hazard analysis (if applicable)
  • Current year COI *
  • Generic bonding letter from vendors surety company stating their current limits, dated within the last 6 months (if applicable) *
  • Current HUB participation plan (if applicable)
  • *Must be updated during renewal period yearly
