HOW DO I PREQUALIFY WITH SAMET?
Samet asks that all subcontractors complete a prequalification application via TradeTapp by Building Connected once a year. This system mainstreams questions, utilizes multi-users to complete the application, and saves all documents on a secure server by AutoDesk.
REQUIRED DOCUMENTATION
- Enterprise business certification proof (if applicable)
- Professional licenses proof (if applicable)
- Minimum of 3 project references
- Current most fiscal year-end financial statement
- Audited or CPA reviewed statements preferred (consists of a balance sheet + income statement) *
- Current year W9
- Letter from your bank stating your relationship
- EMR Verification
- Letter from vendors insurance agency stating their EMR score for the last 3 years *
- 3-year loss run report from vendors insurance agency if EMR is over 1.25 * , if you don’t qualify for EMR, or if business is less than 10 employees
- OSHA 300 & OSHA 300A logs from the last 3 years
- Current safety manual
- Current substance abuse policy
- Job hazard analysis (if applicable)
- Current year COI *
- Generic bonding letter from vendors surety company stating their current limits, dated within the last 6 months (if applicable) *
- Current HUB participation plan (if applicable)
* Must be updated during renewal period yearly
